The Maine Chapter AAP is working in partnership with the Children with Special Health Needs (CSHN) program at the Maine Bureau of Health and the Maine Support Network to expand access to Medical Home resources and tools. 

As part of this work, we have developed a web based survey to ascertain the “medical home-ness” of pediatric practices in Maine. We would greatly appreciate your participation. The survey should take no more than 10 minutes of your time.
