OBH Launches Single Assessment Process for Determining Eligibility for Higher Intensity BH Services

Effective February 2, 2026, the Office of MaineCare Services (OMS) and the Office of Behavioral Health (OBH) launched the new Single Assessment process for determining eligibility for medium- to high-intensity behavioral health services.

The video below shows providers HOW to refer patients to obtain a single assessment for eligibility for medium to high intensity behavioral health services. So if your patient may need in-home therapy, ACT (Assertive community treatment) MST ( multi systemic therapy) or other intensive psychotherapies - where there are often long wait lists this single assessment process will be used to determine if they are eligible. A referral will then be needed to an appropriate organization that can provide the services.

Acentra’s licensed clinicians complete the assessment and determine eligibility on behalf of OMS.

The form is not password-protected. The Single Assessment Referral Link is designed for medical providers—not families or the general public. It should be used when you believe a youth may require a medium- or high-intensity behavioral health service and eligibility needs to be determined through the Single Assessment.

This external link should be used only for children who do not have a BH Care Coordinator or Case Manager, and only by providers who do not use Acentra’s Atrezzo portal.

BH Case Managers, Care Coordinators, and any provider who uses the Acentra Provider Portal (Atrezzo) should continue submitting Single Assessment requests through Atrezzo.
